This application is for people that have never had a CoC, their card has expired or if they are upgrading the level of their ticket

This form will take approximately 15 minutes to complete.

Complete this form if you have never had a CoC or if your card has expired.

You cannot save and return to complete this form later.
You need to have the following files ready: NZQA Record of Achievement and/or NZQA Certificate, passport photo and employer letter of attestation.
If the qualifications were gained less than 6 months ago, you'll also need to supply a Signed Managers Character Reference.
You need your NZQA/NSN numbers. You can complete the application without these, but your application could be delayed.
You need a credit or debit card ready (Visa, Mastercard and American Express accepted) or choose that your employer will be paying by bank payment. Your application won't be processed until we receive the payment.
If you have your CV handy (PDF or Word Doc), and it has enough detail in it, you can upload this instead of completing the on screen work experience table.
